CONSUMERS’ RIGHT TO CHANGE AND/OR CANCELLATION:
Any patient at Restore may cancel or change an agreement without any penalty or further obligation within three (3) business days from signing date. Notice of cancellation or change must be in writing, signed, and sent to Restore by certified or registered mail (so that both parties know that the document was received) or presented directly to the Center Site Director in person. Patients must return with their notice of cancellation/Change Agreement form, relevant documents, and/or any other evidence of relationship with the Center.
Patients may also cancel or change a program with a 30 day written notice; in such case next payment will be processed, if inside 30 days but ongoing payments and program may be cancelled with written notice. Patient must sign the cancellation/change document which can be found at the front desk or below:
