CONSUMERS’ RIGHT TO CANCELLATION:
Any patient at Restore may cancel an agreement without any penalty or further obligation within three (3) business days from signing date. Any services used prior to cancellation must be paid for using per session pricing. Notice of cancellation or change must be in writing, signed, and sent to Restore by certified or registered mail (so that both parties know that the document was received) or presented directly to the Center Site Director in person. Patients must return with their notice of cancellation/Change Agreement form, relevant documents, and/or any other evidence of relationship with the Center.
Patients may also prematurely cancel or change a program with written notice; in such case patient will be charged a one-time cancellation fee of $99.00 for processing this cancellation:
